B. Persyaratan
1. Surat Permohonan.2. Poto kopi KTP (1 lembar) dan Pas photo 4x6 (3 lembar) Pemohon.3. Poto kopi Surat Ijin Bidan (SIB) dari Dinkes Provinsi yang dilegalisir.4. Rekomendasi dari Organisasi Profesi Bidan (IBI) Cabang Kota Palembang.5. Surat persetujuan atasan bila dalam pelaksanaan masa bakti atau sebagai PNS, pegawai swasta dan TNI/ POLRI.6. Rekomendasi dari Pimpinan Puskesmas wilayah setempat.7. Surat Keterangan Berbadan Sehat Asli dari dokter yang memiliki SIP.8. Foto copy ijazah pendidikan kebidanan.
C. Besarnya Tarif / Biaya pelayanan.
Besar Tarif Penyelenggaraan Ijin Praktik Bidan dan Bidan Madya Rp. 0,-
D. Waktu Penyelesaian Pelayanan.
Maksimal 12 Hari kerja terhitung setelah persyaratan dipenuhi.
Catatan :
Ijin Praktik Asisten Farmasi Muda dan Asisten Farmasi Madya berlaku selama 3 Tahun.
